Loan Status Update Form

The Loan Status Update Form can be used to report student loan change transactions on MDHE-guaranteed loans.

You may also use MODEL Direct to post change transactions directly to the MDHE’s system, realtime.
Completed Loan Status Update Forms may be mailed, faxed, or emailed to the MDHE.

• Email procedures. To transmit this form via email, download the form, fill in the applicable fields, and save the completed form. Attach the completed form to an email message to send it to the MDHE (mdheadjustments@amsa.com).
• Fax process. To fax, download and print the form, fill in the applicable fields, and send to: (617) 728-4680. Address the cover sheet to MDHE Call Center Staff.
• Mailing address. To mail, download, print, and complete the form. Mail to: MDHE c/o ASA, Loan Processing Manager, 100 Cambridge Street, Suite 1600, Boston, MA 02114.
